Once we have added the add-on, we need to set it up for being functional with our form. When we open the add-on, it will ask us whether we want to open it or go to the help section.

We can set up the signature on this form after we login to this add-on , it’s not complicated at all. It’s just to check our membership. We will go for multiple signatures because we will need different patients.

We can set the limit of signatures per form and we can also password protect these signatures so that they won’t be misused by anyone else. We set the limit of signatures to 2 because we have information for 2 individuals. One is the patient and the other one is a family member or emergency contact.

In the response tab, we can find a link to the sheet button and press it to add responses to a spreadsheet automatically. We can create new spreadsheets or we can also choose existing spreadsheets as well.

Once we are done with making spreadsheets, we can go back to add-on and confirm again. Most of the time, there are no errors but sometimes add-ons are not compatible with file upload type questions. You can make another form for medical history and use a simpler form for consent.
