Now as you can see, there are buttons that say “Go to forms”. There are two of them, click on either one of them to go to the Forms to create a Google Form.
Here, you can see your forms that you’ve made already. Or you can select from one of the templates provided by Google. You may choose one of the provided templates and customize it to your liking as it will save up a lot of precious time. Various templates are shown below:
However, we will be starting a completely new form and will make it from the blank.
Click on “Blank” to choose a blank template as:

Upload the PDF file on Google Drive
To add a PDF file to Google Forms, we need to upload it to our Google Drive first. For that, go to Google Drive’s website. Here I have added the hyperlink. You can follow it to go to Google Drive.

Now let’s upload the PDF to our Drive first. To upload the PDF file, click on “New” at top left of the screen as shown below:
Click on New will give you a few options.
These include,

– File Upload
– Folder Upload
– Google docs
– Google sheets

More also includes some more options like Google drawing, Maps, Sites, Apps Script and Jamboard, etc.
Click on “File Upload” as we are going to upload a PDF file.
A pop up screen will appear to locate the file that you want to upload.
Locate the PDF file in your PC and select it. You can also see a little preview of the file as shown below:
After selecting the file, click “Open” as:
Now the file is uploaded to your Google Drive. You can find it in your drive by simply typing the name of the file in the search box.
Copy the PDF file link from Google Drive
Now all you’ve to do is copy the link of the file to use it for our Google Form.
You can do it by right clicking on the file and clicking “Copy link”.
Now, the link to your PDF file is copied to the clipboard. All that’s left now is to see how to add it to the Google Form.

Now the file that we uploaded to Google Drive contains some key information about these awards that is fitting for the description. So Let’s add the link of the file in the Description of the form.

Just to make it look more appealing, let’s make a Hyperlink.
Select the text that you want to make Hyperlink and click on the “Link” button.
Simply add the link in “Link to” field of pop up screen and click “Ok” as:
You may remove the earlier link now. As you see, the text that we selected is hyperlink now. Clicking on the Hypertext will take our responders to the PDF file.

- If a form owner wants to add more and more description or text and pictures to his or her form that destroys the alignment of the form and takes up too much space, he or she can add it all to a PDF file and add this PDF file to the Google Form.
- Make sure that you are allowing everyone to view the PDF in Google Drive. If it is in restricted mode, then although people can reach that link, they would not be able to view the PDF file.
- It is not necessary to upload the PDF file in your own Google account. If you have a PDF file link of another person’s Google Drive that can be publicly used, you may use that in your form.
